Integrated Neighbourhood Teams
What are integrated neighbourhood teams?
Integrated Neighbourhood Teams (INTs) bring together professionals from across the NHS, local councils, mental health services, community organisations and voluntary groups to work together as one local team around the needs of local people and communities.

This approach is part of the NHS 10 Year Health Plan and London’s wider Neighbourhood Health Service strategy, which aims to shift healthcare:
- from hospital to community
- from analogue to digital
- from treatment to prevention
The goal is to make care more joined-up, proactive and personalised, helping people access the right support earlier and closer to home.
How do neighbourhood teams work?
Neighbourhood teams connect services that have traditionally worked separately, including:
- GPs and primary care
- community healthcare services
- hospitals
- mental health services
- adult and children’s social care
- housing teams
- pharmacists
- voluntary and community organisations
By working together as one team, services can better support people with complex or long-term needs, improve communication between organisations, and provide more coordinated care around the whole person, not just individual conditions.
What does this mean for local people?
The aim is for people to experience:
- more joined-up support
- less repetition between services
- easier access to care and advice
- more support within local communities
- earlier intervention and prevention
- better support to stay healthy and independent for longer
Across south east London, neighbourhood working is currently focusing on:
- people living with multiple long-term conditions
- older adults living with frailty
- children and young people with complex needs
Neighbourhood working in Southwark
Southwark has five neighbourhood areas:
- Peckham and Nunhead
- Dulwich
- Camberwell and Walworth
- Borough
- Bermondsey and Rotherhithe
Local teams are working together to improve care for residents through stronger partnership working, prevention, community support and reducing unnecessary hospital admissions.
This work also supports wider priorities around reducing health inequalities, improving population health and building services around the needs of local communities.
Strategy and the NHS 10 year health plan
Integrated Neighbourhood Teams are a key part of the NHS and London vision for a future “Neighbourhood Health Service”. This is a long-term transformation programme designed to create a more sustainable, equitable and community-based health and care system over the next 10 years.
In London, the Target Operating Model (TOM) sets out how organisations across health, care and the voluntary sector will work together to deliver this vision consistently across every neighbourhood.
The strategy recognises that improving health outcomes is not only about healthcare services, but also about working together with communities to address the wider causes of poor health and inequality.
