Clinical pharmacists have been working in general practice for a number of years now. They are highly qualified experts in medicines and can help people in a range of ways. They clinically assess and treat patients, using their expert knowledge to improve patient care and patient outcomes. They support people with long-term conditions, considering their health needs and medications.
They are based within each of the practices in South Southwark and a core part of their team.
