HSJ awards - Multimorbidity Model of Care Programme

In November the Multimorbidity Programme of Care team attended the HSJ Awards Ceremony, where their multimorbidity model of care was shortlisted for both Primary and Community Care Initiative of the Year and Integrated Care Initiative of the Year. Although not selected as winners, the shortlist recognition reflects the team’s collaboration, innovation, and commitment to improving outcomes for people with multiple long-term conditions. Finalist certificates have been shared as a mark of this achievement, and the team continues to gain national attention for its approach, which is influencing neighbourhood health implementation and inspiring interest from organisations across the country.

The programme remains focused on enhancing the health and wellbeing of the south east London population while supporting staff through effective, sustainable models of care.

Updated patient experience survey results, reflecting feedback from 126 individuals who attended complex case management clinics, highlight the positive impact of the team’s work.
